Disease State Management

Modeled after the ground breaking "Asheville Project", a group of CarePro clinical pharmacists have developed a structured disease management program. These programs have presented the greatest return on investment for employer groups as studies show 20% of the employee base generates 80% of the health insurance claims for employer group health plans.

CarePro's Goal:

  • Improve employee health and quality of life.
  • Reduce the risk/cost of chronic health complications.

Participants in the Disease State Management program meet monthly with a CarePro pharmacist for continued disease state and medication education, monitoring of health measures, setting healthcare goals, establishing plans to meet goals and playing the role of a health mentor and a communicator between patient and physician. The benefit of using a pharmacist for DSM programming is to utilize a health professional who has experience in wellness prevention and disease state education.

Disease State Management Aggregate Report

CarePro Health Risk Management has developed a detailed program for continued monitoring and follow-through to show improvement in the employee health and employer's return on investment. A disease state management aggregate report is provided to the employer on an annual basis to help evaluate progress of the participating employees.

Optional Health Coaching

To compliment monthly pharmacist meetings, CarePro Health Risk Management would like to offer an additional service that will allow the participant to receive a telephone call from a CarePro health coach between monthly pharmacist visits. The purpose of this additional contact with the employee is to encourage compliance with clinical and behavioral change recommendations that the pharmacist discusses with the participant during monthly visits.
